New Patient Registration
The Practice will only accept new patients from the area defined by the following postcodes:
BN1 1xx, BN1 4xx, BN2 0xx, BN2 1xx, BN2 2xx, BN2 3xx and BN2 5xx, BN2 9xx.
If you live at an address or move to an address outside of these areas, you need to register with another practice.

Your Registration
You can now register with us online! Click here to complete the online form.
If you are unable to register online, we ask that you print or collect the following forms from Reception. These will need to be completed in full in order to be accepted. We also need you to produce two forms of identity when you attend the surgery to hand your forms in:
One with your picture on it, i.e. a passport or driving licence, and one proof of your new address i.e. utility bill or bank statement.
Reception staff will be able to check your registration form and ID during our opening hours. Registrations are usually completed on a Tuesday morning but can take between 5-10 working days to be processed. We will text you when we have processed your registration.

Once you've been registered...
Patients are registered to an individual GP within the practice but we will make appropriate arrangements if you need to be seen and your GP is not available.
You may also express a preference to be seen by a particular doctor or nurse for all or part of your care. We will try to comply with your request if possible.
Medical treatment is available from the date of registration (only where immediate and necessary treatment is required and appointment availability allows). Registration usually takes up to 5 working days.
If you urgently need to see a doctor when you hand your forms in, please speak to Reception for further information (they may sign post you to another service if appropriate).
