From 31st March 2018 you will no longer be able to make appointments at both locations. You are required to make a decision on where you would like your future care to be. Please contact the surgery to inform us of your choice.

If you live within our practice boundaries you may apply to join our practice list.

We would request that you complete the pre-registration form online by selecting the "Registering Online" tab above and completing the form, or alternatively printing the form which can be found at the bottom of the same page, completing it and bringing it to the surgery.

Please note, we are not able to deal with new patient registrations before 11:00. We would ask you to bring along two forms of identity. One with your picture on it, i.e. a passport or driving licence, and one proof of your new address i.e. utility bill or bank statement.

Patients are registered with the practice not an individual GP. Your medical card will be issued in the name of the doctor who will supervise your care; however you may also express a preference to be seen by a particular doctor or nurse for all or part of your care. We will try to comply with your request if possible.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Practice Boundaries

The Practice will only accept new patients from the area defined by postcodes BN1 1xx, BN1 4xx, BN2 0xx, BN2 1xx, BN2 2xx, BN2 3xx and BN2 5xx.

If you move to an address outside this area, we will to ask you to re-register with another practice.

Online 'Pre-Registration' With The Practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.  You will be required to attend the surgery to complete this application.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us - this will save you having to complete this when you attend the surgery. The link to this is below. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
