                                                        Urgent Patient Notice 

Have you informed us of which practice you would like to be seen at after the 31st March 2018? 

From 31st March 2018 you will no longer be able to make appointments at both locations. You are required to make a decision on where you would like your future care to be. Please contact the surgery to inform us of your choice.

 

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Passport signing
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

A special appointment is needed for medicals so please speak to the receptionist to arrange this. You will be asked to pay a £20 deposit in cash when booking a medical. Once you have attended the medical this will be either refunded or deducted from the final account. Please note if you fail to attend the deposit will not be refunded.
